SCHOOL CATECHIST SURVEY

Dear Teachers,

We are assessing the catechetical needs of our schools. Please assist us by providing the following information. Please submit your form no later than May 13. Thank you and God bless you!

	If served as a catechist in a religious education program. Select the parish.
*Please fill out*	ONLY ONE of the following sections.

	1. I hold a current Catechist Certification from Diocese of Nashville.
	If you checked #1, indicate when your certification expires.

	2. My certification has expired and needs to be renewed.
	*If you checked #2, please submit an Application for Certification Renewal Form and return all necessary documentation to the Office of Catechetical Formation.*

From the Diocese of	3. I hold a current Catechist Certification from another diocese and need to apply for Nashville certification.
	If you checked #3, please submit an application for CERTIFICATION TRANSFER Form and return all necessary documentation to the Office of Catechetical Formation.

From the Diocese of	4. I began a program for Catechist Cetification in an other diocese and need to complete my formation with the Diocese of Nashville.
	*If you checked #4, please submit an Application for TRANSITIONAL CATECHIST Form and return all necessary documentation to the Office of catechetical Formation.*

	5. I am not pursuing a catechist certification at this time.
